This is an invitation to participate in a survey for a research project about improving hospital discharge for carers and older adults

Carers NT and Monash University are seeking at least 30 carers of older adults living in the community to take part in a survey for a research project.

The research project aims to identify carers’ experiences and needs in supporting older adults who are discharged from hospital to home
